Schwannoma retrorrectal gigante: una entidad infrecuente en la práctica de Cirugía General.

Giant retrorectal schwannoma: an infrequent entity in general surgery practice.


Resumen

Introducción: Las tumoraciones retrorrectales son muy infrecuentes, y se clasifican en 5 categorías (congénitas, neurogenicas, óseas, inflamatorias y misceláneas), siendo las congénitas las más frecuentes.

Descripción del caso: Mujer de 63 años con lesión retrorrectal cuya aparición se asocia a un traumatismo, y que se interpreta como hematoma cronificado. Tras RMN que describe lesión quística homogéneamente hipertintensa en T1, y punción que obtiene líquido serohemático, informado por AP como "contenido de quiste benigno" nos la remiten por persistencia de la lesión y molestias a la sedestación. Se procede a extirpación completa mediante abordaje posterior y el informe anatomopatológico describe Schwannoma celular encapsulado. La paciente presentó postoperatorio sin incidencia, con alta al día siguiente. Revisión tras 3 meses sin incidencias.

Discusión: Los tumores retrorrectales son infrecuentes, y suelen publicarse habitualmente como casos únicos o series muy escasas. Según ellos, los Schwannomas representan entre 0,4 y 15% del total. Más frecuentes en mujeres, presentan crecimiento lento. El diagnóstico por imagen suele precisar de una RM y la biopsia prequirúrgica no suele aconsejarse.

El tratamiento es quirúrgico, mediante la exéresis completa, generalmente por vía posterior, aunque por encima de S3 es posible abordarlas por vía abdominal, abierta o laparoscópica.

Abstract

Background: Retrorectal tumors are very rare, and are classified into 5 categories (congenital, neurogenic, bone, inflammatory and miscellaneous), being the congenital ones the most frequent.

Case description: A 63-year-old woman with a retrorectal lesion whose appearance is associated with trauma, and interpreted as a chronic hematoma. After MRI that describes homogeneous hypertintensive cystic lesion in T1, and percutaneous puncture that obtains serohematic fluid, reported by pathologist as "benign cyst content", patient was referred to surgeon, due to persistence of the lesion and discomfort when sitting. Complete removal was performed by posterior approach and the pathological report described encapsulated cell Schwannoma. The patient presented no postoperative complications, and was discharged after one day. Revision after 3 months did not report incidents.

Discussion: Retrorectal tumors are rare, and are usually published as single cases or very small series. In them, the Schwannomas represent between 0.4 and 15% of the total. More frequent in women, they present slow growth. Imaging usually requires an MRI, and pre-surgical biopsy is not usually advised. The treatment is surgical, by complete excision, usually by posterior approach, although above S3 it is possible to approach them by abdominal, open or laparoscopic.
