El cirujano ante la patología mamaria, técnicas diagnósticas, clasificación BI-RADS®

Resumen

Exponemos la evolución histórica del papel de la cirugía en el tratamiento del cáncer de mama, destacando tres eventos fundamentales: la teoría mecanicista y el tratamiento radical de Halsted, la teoría sistémica con la implantación del tratamiento conservador de Fisher y Veronesi y la desescalada en la estadificación ganglionar con implantación de la BSGC tras el ensayo NSABP-B32. Revisamos el estado actual de la docencia de la patología mamaria en la formación de los cirujanos generales en España y Europa. Exponemos las características de las unidades multidisciplinares de patología mamaria y los criterios de acreditación de SESPM y EUSOMA. Analizamos las diferentes técnicas de imagen y sus indicaciones en la patología mamaria y la clasificación BIRADS.

Abstract

We present the historical evolution of the role of surgery in the treatment of breast cancer, highlighting three fundamental events: the mechanistic theory and the radical treatment of Halsted, the systemic theory with the implantation of the conservative treatment of Fisher and Veronesi and the de-escalation in the Lymph node staging with SLNB implantation after the NSABP-B32 trial. We review the current state of the teaching of breast pathology in the training of general surgeons in Spain and Europe. We present the characteristics of the multidisciplinary units of mammary pathology and the accreditation criteria of SESPM and EUSOMA. We analyze the different imaging techniques and their indications in breast pathology and the BIRADS classification.
