Cirugía en hospital comarcal. ¿Qué se hace? ¿Qué no se hace? ¿Por qué?

Resumen

La mejora asistencial de los hospitales comarcales ha favorecido que cada vez mayor número de patologías quirúrgicas, y más complejas, puedan tratarse en ellos.

Sería oportuno plantear la posibilidad de actualizar la cartera de servicios de Cirugía General y del Aparato digestivo (CGAD) de los hospitales comarcales públicos en Andalucía.

En teoría, la oferta de procedimientos por niveles asistenciales, constituye una herramienta que permite, además de la correcta identificación de la actividad realizada, el establecimiento de procedimientos de referencia según la complejidad y puede constituir un elemento más a la hora de programar las actividades formativas correspondientes a cada nivel asistencial. Esta ordenación no sería limitante pudiendo experimentar ajustes en el tiempo en función de los avances científicos en cada ámbito.

Se presentan los resultados de una encuesta a los servicios de CGAD de los diecisiete hospitales comarcales del Sistema Sanitario Público de Andalucía sobre los procedimientos quirúrgicos de la especialidad que se realizan, de forma programada, en su centros, así como los razonamientos de sus respuestas.

Reflexiones:

• La actividad quirúrgica programada de los servicios de CGAD en los distintos hospitales comarcales de Andalucía es heterogénea. Cada centro realiza unos procedimientos que en algún otro(s) hospitales de su mismo nivel no se realizan y viceversa.

• Todavía no existe en CGAD un sistema que acredite a los centros para la realización de determinados procedimientos, aunque existan estudios que avalan su conveniencia.

• Homogeneizar la cartera de servicios de CGAD, en los distintos hospitales comarcales de nuestra comunidad es un tema controvertido: ¿Es aconsejable? ¿Es limitante?

• Definir la actividad de los servicios de CGAD, de los hospitales comarcales: ¿Se debe? ¿Se puede? ¿Tiene ventajas? ¿Acarrea conflictos?

• Es de destacar el papel relevante de nuestros hospitales comarcales en el tratamiento de las distintas patologías, si bien todavía existen diferencias sustanciales en cuanto a medios diagnósticos y terapéuticos que en un futuro deseamos sean solventados al aumentar sus recursos.

Abstract

The improvement of care of the regional hospitals has favored that an increasing number of surgical pathologies, and more complex, can be treated in them.

It would be opportune to raise the possibility of updating the portfolio of services of General Surgery and Digestive System (CGAD) of public district hospitals in Andalucía.

In theory, the offer of procedures by levels of care, is a tool that allows, in addition to the correct identification of the activity carried out, the establishment of reference procedures according to complexity and can be a further element when programming training activities corresponding to each level of care. This ordering would not be limiting, being able to undergo adjustments over time depending on the scientific advances in each area.

The results of a survey of the CGAD services of the 17 regional hospitals of the Public Health System of Andalucía are presented on the surgical procedures of the specialty that are carried out, in a programmed manner, in their centers, as well as the reasoning of their answers.

Reflections:

• The programmed surgical activity of the CGAD services in the different regional hospitals of Andalusia is heterogeneous. Each center performs procedures that are not performed in other hospitals of the same level and vice versa.

• There is still no system in CGAD that accredits the centers to carry out certain procedures, although there are studies that support their convenience.

• Homogenizing the portfolio of CGAD services in the different regional hospitals of our community is a controversial issue: is it advisable? Is it limiting?

• Define the activity of the CGAD services, of the district hospitals: is it due? Can it be? Does it have advantages? Does it lead to conflicts?

• It is important to highlight the relevant role of our regional hospitals in the treatment of different pathologies, although there are still substantial differences in terms of diagnostic and therapeutic means that in the future we want to be solved by increasing their resources.
