Acreditación docente en cirugía general y del aparato digestivo en un hospital comarcal

Teaching accreditation in general and digestive surgery in a district hospital


Resumen

De las 6.513 plazas de MIR adjudicadas este año, el 2,9% han sido de Cirugía General y del Aparato Digestivo y de éstas, el 32,27% lo han sido en hospitales fuera de las capitales de provincia, lo que no quiere decir que se traten de hospitales de primer nivel.

En Andalucía, aunque hay más hospitales privados que públicos, el 73,2% de las camas son públicas y la formación MIR en Cirugía General y Digestiva se centra exclusivamente en hospitales públicos, y dentro de éstos en los hospitales de primer y segundo nivel (hospitales regionales y hospitales de especialidades). Nuestro hospital, es el único hospital de primer nivel o comarcal de Andalucía acreditado para la formación MIR en Cirugía General y Digestiva.

Hasta conseguir esta acreditación, muchos han sido los pasos que han habido que dar, y que prácticamente se iniciaron poco después de la apertura del hospital en 1988. Una breve historia de las bases que nos motivaron a solicitar dicha acreditación, así como como las distintas fases de la misma (solicitud, auditoria, dictamen, etc.), son expuestas en esta ponencia. Así mismo se detallan los requisitos necesarios para acreditar a un Servicio de Cirugía como Unidad Docente.

Por último, se presenta un estudio comparativo entre la patología quirúrgica y el número de intervenciones que debería haber realizado un MIR de quinto año al acabar su periodo de formación, según la Comisión Nacional de la Especialidad de Cirugía General y Digestiva, con las realizadas por los tres MIR que ya han finalizado su formación en nuestro hospital, y con un MIR de nuestro hospital de referencia de segundo nivel.

Como conclusión, creemos que la formación de los MIR de Cirugía General y Digestiva en un hospital comarcal, como es el nuestro, no difiere en mucho de la recibida por los MIR en hospitales de mayor nivel. Si bien es verdad que nuestros residentes realizan más intervenciones en cifras absolutas.

Abstract

The 2.9% of the 6513 vacancies of the Residency Program awarded this year, have been in General and Digestive Surgery and, of these, the 32.27% have been in hospitals outside the county capitals, which doesn´t mean that these were a third level hospitals.

In Andalusia, although there are more private than public hospitals, the 73.2% of the beds are public and consequently, training in General and Digestive Surgery Residency Program focuses exclusively on public hospitals, and almost exclusively in first level hospitals (Regional Hospitals) or second level (Hospitals of Specialties). Our Hospital is the only district hospital or in first level of Andalusia accredited for training in General and Digestive Surgery.

To achieve this accreditation, many steps have been given and we practically started immediately after the Hospital opening in 1988. A brief history of the bases that motivated us to request this accreditation, as well as the different phases of this (request, audit, opinion), are exposed in this report. Also we detail the requirements necessary to accredit a Department of Surgery as a teaching unit.

Finally, a comparative study of the surgical pathology and the number of interventions that should have made a fifth year resident doctor at the end of their training period, according to the National Commission of the Specialty of General and Digestive Surgery is presented, with the performed by the three resident doctors that have already completed their training at our Hospital, and with another resident doctor of our second level Reference Hospital.

In conclusion, we believe that a Residency Program in General and Digestive Surgery at a district hospital, like ours, doesn’t differ greatly on the received by the resident doctors at higher-level hospitals, although it is true, that our residents perform more interventions in absolute figures.
